Please fill out our intake form so our staff can contact you, assess your needs, and refer you to available resources. We are committed to helping anyone affected by the events of November 21, 2021, including those who live in communities outside of Waukesha or Wisconsin.
The purpose of this form is to establish you with UWRC and to help us understand your needs. This form will take about five minutes to complete. Your responses will help us determine how we can better serve you. After completing this intake form, a Service Navigator will reach out to you with next steps.

Financial Assistance
Victims and survivors impacted by the Waukesha Parade Incident may be eligible for financial assistance for the out-of-pocket cost for medical or mental health treatment, funeral expenses, lost wages, and other expenses directly related to the crime.
