What to Expect: Contacting the United for Waukesha Resiliency Center

So often in life I procrastinate on making the calls, or doing the things I need to do because I just don’t know what to expect and this causes some anxiety. We don’t want that to be the case for you in reaching out to the United for Waukesha Resiliency Center to get connected with services! 

The first step:  

If you are interested in one of our scheduled groups or activities, simply complete the registration form! A Service Navigator will send a reminder e-mail with information on location just prior to the event/group. 

Or… 

In order to initiate services such as individual or family therapy, you can either call a Service Navigator at 262-522-0243, or complete an interest form on our website: www.unitedwaukesha.org. If you decide to complete the interest form (the link is on the bottom of the home page and at the top of our programs tab), you may indicate how you prefer to be contacted: phone call, e-mail or text messaging. 

The next step: 

Once we receive your call or interest form, the next step is to complete an intake form. This can be completed over the phone, in-person, or a link can be e-mailed or texted to you. This will provide the Service Navigators with the information required to make referrals on your behalf (such as insurance information and desired services). 

The Service Navigators will seek out the appropriate therapist and ensure that it will either be covered under insurance or arrange for the United for Waukesha Resiliency Center to be billed for the services. A digital release of information form can be emailed or texted to you allowing the Service Navigators to coordinate treatment. Once the basic arrangements are established, you will be connected with the person or clinic with the forms they may need completed and an intake appointment will be scheduled for you. 

After you are established with treatment services, the Service Navigators will periodically check in with you on how things are going and see if you are in need of any additional assistance!
